You have 2 options with this form. A) Just print the form and take it to your physician. B) Type in your childs info (except the doctors part), print the form and take it to your physician. | Once you obtain your physical form take it to your physician and get your physical. Your form must be completed by the parent/guardian and then completed by your physician. Return the form to Mr. Baum's office.
